Partnered with Yolo County and Dignity Health, Friends continues to support YADHC to provide much needed services to elderly and disabled members of the community. As an all-volunteer non-profit organization, Friends continues to carry on the spirit of its original members by honoring their goals and following our mission statement to be “committed to promoting the health, dignity, and independence of frail older and disabled adults and their caregivers by supporting the Yolo Adult Day Health Center through fundraising and community awareness.

Yolo Adult Day Health Center assists individuals who experience any of the following:
  1. Have trouble with everyday tasks
  2. Need assistance in managing medication
  3. Need ongoing physical, occupational or speech therapy
  4. Are isolated, depressed, confused or suffering memory loss due to disease or a related condition
  5. Are isolated, depressed, confused or suffering memory loss due to disease or a related condition, OR
  6. Family caregivers need support

Adult Day Services Programs

Adult Day Health Care

A medical model program providing a team approach to managing complex chronic health conditions that impact independent living. Team members include participant's physician, nurses, social workers, rehabilitation specialists, recreation therapists, dietician and mental health professionals along with the participant and his/her caregivers. This dynamic interdisciplinary approach provides comprehensive support that includes all aspects of the individual’s life including home environment.

Adult Day Health Care

Nursing and personal Care

The Center’s registered nurses work closely with each person’s primary care physician to monitor complex chronic conditions, assist with medications, and provide skilled interventions to address blood pressure, blood sugar, pain and much more.  Additionally, nurses assist with accessing other health care services. Nursing assistants, under the supervision of RN’s provide personal care support such as toileting, feeding, hygiene, grooming and even showering.

Rehabilitation

 Physical, occupation and speech therapy programs provide ongoing treatments to assist with maintaining, regaining and even improving a participant’s functional, cognitive, speech and swallow abilities.   

Social Work

 Social work staff assist individuals and their families with planning for long term care needs, provide counseling regarding change, transitions, loss and help with problem-solving financial and legal issues, accessing community resources, and providing ongoing caregiver support.





Therapeutic Activities

Center staff work closely with each person to develop personalized activity plans and structured activities that address individual preferences and skills. Attention is given to each person’s interests related to socialization, creativity and self-improvement. 

The focus is empowering a person to engage and create which helps maintain a sense of control. In many ways, we like to think of ourselves as coaches and facilitators more than care providers. Participants are provided with a hot meal consistent with individual dietary restrictions daily. A dietary assessment is completed on each participant and counseling provided to ensure optimal nutritional status.

Transportation

Transportation to and from the Center is provided by the program as needed and requires the upkeep and replacement of W/C accessible vans and buses.  Over the years, Friends of ADHC has been responsible for the purchase of four wheel chair vans and buses along with support from the CA Dept. of Aging and Woodland Healthcare Foundation. 





Specialized Dementia Services
The CASA program is a specialized program within a program, targeting the needs of individuals with a chronic neurocognitive disorder, especially Alzheimer’s disease, the most common form of dementia. Besides therapeutic activities, CASA provides needed respite to caregivers and offers caregiver support groups and other assistance for caregivers in coping with the multiple challenges of living with/caring for a loved one with dementia.
 

Adult Day Care

A social model program offering program participants a stimulating and safe environment with staff trained in dementia care and family caregiver support. Center also provides specialized staff training to meet the needs of individuals with moderate to severe dementia, who require additional assistance to benefit from the Center.

Both programs include therapeutic activities focusing on:

  • Creative expressions through arts and crafts
  • Musical entertainment and group sing-alongs
  • Mental stimulation games
  • Stretching and gentle exercise (ex: Chair yoga)
  • Discussion groups about books, films, or current events
  • Holiday and birthday celebrations
  • Nutritious meals and snacks

Caregiver Support Program

 The caregiver support program provides direct assistance to community caregivers by offering care coordination, counseling, support groups, workshops and consultation and referral for grief, coping, caregiver burden, caregiving skills and assistance with alternative living situations. 

Currently, the center offers a caregiver support group monthly on the second Tuesday from 6:30-8pm. The group continues via zoom conference calls.

Senior Transportation Voucher Program (STVP)


STVP offers vouchers to any Yolo County Participants over the age of 60 who relies on YOLOBUS Special or Davis Community Transit for transportation to medical appointments, shopping, or other needs.


This is made possible via a funds from Agency on Aging Area 4.
