About Yolo Adult Day Health Center

The diverse programs of Yolo Adult Day Health Center promote the well-being, dignity and self-esteem of an individual. Their goal is to maximize independence, improve management of chronic symptoms, prevent hospitalization and/or premature nursing home placement and provide support and relief to caregivers.

Yolo Adult Day Health Center

assists individuals who...


  • Have trouble with everyday tasks,
  • Need assistance in managing medication,
  • Need ongoing physical, occupational or speech therapy,
  • Are isolated, depressed, confused or suffering memory loss due to disease or a related condition and
  • Have family caregivers who need support.*


*Caregiving for a loved one carries with it significant risk for poor health outcomes due to 24/7 demands. 


Providing education on dementia, 1:1 counseling and group support can help relieve the sense of isolation and overwhelming anxiety so common among caregivers.

Specific Programming Includes:

Adult Day Health Care

A medical model program that provides an array of services to restore, preserve and improve overall health and well-being, using a team approach to manage complex chronic health conditions that impact independent living. Team members include the older or dependent participant and his/her physician, nurses, social workers, rehabilitation specialists, recreation therapists, dietician, and mental health professionals and his/her primary caregiver. This dynamic, interdisciplinary approach improves health outcomes by providing comprehensive support that addresses all aspects of the individual’s ability to live safely in his/her home and community. 




Nursing and Personal Care

The Center’s registered nurses work closely with each person’s primary care physician to monitor complex chronic conditions, assist with medications, and provide skilled interventions to address blood pressure, blood sugar, pain and much more.  Additionally, nurses assist with accessing other health care services. Nursing assistants, under the supervision of RN’s provide personal care support such as toileting, feeding, hygiene, grooming and even showering.


Rehabilitation

Physical, occupation and speech therapy programs provide ongoing treatments to assist with maintaining, regaining and even improving a participant’s functional, cognitive, speech and swallow abilities. 

Nutrition Services

Participants are provided with a hot meal consistent with individual dietary restrictions daily.  A dietary assessment is completed on each participant and counseling provided to ensure optimal nutritional status.



Social Work

Social work staff assist individuals and families with planning for long term care services, provide counseling regarding change and transitions, problem-solving financial and legal issues, accessing community resources and providing ongoing caregiver support group.

Therapeutic Activities

Center staff work closely with each person to develop personalized activity plans and structured activities that address individual preferences and skills. Attention is given to each person’s interests related to socialization, creativity and self-improvement. The focus is empowering a person to engage and create which helps maintain a sense of control. In many ways, we like to think of ourselves as coaches and facilitators more than care providers.

Transportation

Transportation to and from the Center is provided by the program as needed and requires the upkeep and replacement of vans and wheelchair accessible buses. Over the years, Friends of ADHC have generously purchased four vans and buses along with support from the Agency on Aging Area 4 and Woodland Healthcare Foundation. 



Specialized Dementia Services

The CASA Program, is a specialized program within a program, targeting the needs of individuals with a chronic neurocognitive disorder, especially Alzheimer’s disease, the most common form of dementia. Besides therapeutic activities, CASA provides needed respite to caregivers and offers caregiver support groups and other assistance for caregivers in coping with the multiple challenges of living with/caring for a loved one with dementia.

Adult Day Care

A social model program offering program participants a stimulating and safe environment with staff trained in dementia care and family caregiver support. Participants engage in Individualized therapeutic activities designed to promote socialization and enhance physical and mental function without a strong focus on rehabilitation or medical monitoring.  The Center also provides specialized staff training to meet the needs of individuals with moderate to severe dementia, who require additional assistance to most benefit from the program.

Caregiver Support

The caregiver support program provides direct assistance to community caregivers by offering care coordination, counseling, support groups, workshops and consultation and referral for grief, coping, caregiver burden, caregiving skills and assistance with alternative living situations.  Currently, the center offers a caregiver support group monthly on the second Tuesday from 6:30-8pm.  The group continues via zoom conference calls.   Call the Center at (530) 669-3700 for information on how to connect or any other question about our program.

Senior Transportation Voucher Program (STVP)

STVP offers vouchers to any Yolo County Resident over the age of 60 who relies on YOLOBUS Special or Davis Community Transit for transportation to medical appointments, shopping, or other needs. This is made possible via a funds from Agency on Aging Area 4.
