About Yolo Adult Day Health Care

The diverse programs of Yolo Adult Day Health Center promote the well-being, dignity and self-esteem of participants. The goal is to maximize independence, improve management of chronic symptoms, prevent hospitalization and/or premature nursing home placement and provide support and relief to caregivers.

  • Require ongoing physical, occupational, or speech therapy,
  • Experience isolation, depression, memory loss, or confusion due to illness, and
  • Have family caregivers who need extra support.



Why caregiver support matters:

Caring for a loved one around the clock can take a serious toll on a caregiver’s health. To help, the Center offers education on dementia, one-on-one counseling, group support, and resource assistance. These services reduce stress, ease feelings of isolation, and connect caregivers with the tools they need to cope.

Specific Programming

Nursing & Personal Care

The Center’s registered nurses partner with each participant’s primary care provider to help manage chronic health conditions. They provide support with medications and skilled care for issues such as blood pressure, blood sugar, and pain management, as well as other health needs. Nurses also help participants connect with additional health care services when needed. Nursing assistants, under RN supervision, provide personal care such as toileting, feeding, hygiene, grooming, and showering.

Therapeutic Activities

Center staff work with each participant to create personalized activity plans that reflect their unique interests, skills, and goals. Activities are designed to encourage socialization, creativity, and personal growth. The focus is on empowering participants to stay engaged and maintain a sense of control in their daily lives. In this way, staff see themselves not just as care providers, but as coaches and facilitators who support and inspire.

Specialized Dementia Services

The CASA Program is a specialized program within the Center that focuses on individuals with chronic neurocognitive disorders, especially Alzheimer’s disease, the most common form of dementia. In addition to therapeutic activities for participants, CASA provides respite for caregivers and offers support groups and other resources to help caregivers manage the many challenges of caring for a loved one with dementia.

Rehabilitation

Physical, occupational, and speech therapy services help participants maintain, regain, and even improve their mobility, thinking skills, speech, and swallowing abilities.

Nutrition Services

Each day, participants receive a hot meal tailored to their individual dietary needs. Every participant also receives a dietary assessment, along with counseling, to support their health and ensure good nutrition.

Social Work

Social work staff support participants and their families by helping them plan for long-term care needs, adjust to life changes, and navigate financial or legal challenges. They also connect families with community resources and lead ongoing caregiver support groups.

Transportation

Transportation to and from the Center is provided by the program as needed and requires the upkeep and replacement of vans and wheelchair accessible buses.
